Start Point: Talk to Juliet, located in the house just outside of West Varrock
Requirements: Cadava Berries (can be obtained from a Cadava Bush, located outside Varrock, between the Dark Wizards and the mine)
To begin this quest, talk to Juliet in her house just outside of West Varrock. It's easy to see - it's quite large. Juliet should be moping around on the second floor, on the balcony.

When you talk to Juliet, she'll ask you about her love, Romeo. She asks you to take a message to him. She seems a bit needy. Run to Romeo in Varrock's Square and give him the message - if he isn't standing by one of the stalls, he'll be around somewhere; try the General Store.

When you find Romeo, he'll read the message from Juliet, who seems to think Romeo sent you to her to tell her that Romeo is still in love with her, which they seem to be though they aren't very smart. However, it would seem that Juliet's father opposes their relationship. In fact, her father will kill Romeo if he sees him.
They put their hope in Father Lawrence, who Romeo remembers after long last. When Romeo is done talking, head north east until you come to the church. Father Lawrence will be inside somewhere.

After interrupting the Father's sermon, you'll inform him about the situation. He comes up with the bright idea to make a potion to make Juliet appear dead, after which Romeo can collect her from her crypt and they can live in peace (apparently no one is thinking of the effect this might have on Juliet's father).
Father Lawrence will tell you to talk to the Apothecary about making the potion. From the church, head south west, past Varrock Square and past the Clothes Shop until you see a little dirt path that leads to a square building (on the mini map it is indicated by a little potion icon). Inside is the Apothecary.

Talk to him and you will explain the situation. For the Cadava Potion he is to make, he has everything he needs but Cadava berries. If you already have some, talk to him again; otherwise, head southeast, out of the city, past the Dark Wizards, and into the small forest. Near the mine, you will see a Cadava Bush - pick it to get some Cadava Berries. Bring them back to the Apothecary and he will make you a Cadava Potion.

When you have the potion, head back to Juliet's house and talk to her - you'll explain the situation. Her cousin will come out on the balcony to make things look more real, and eventually Juliet will drink the potion and keel over. Her cousin will eventually get things right and yell, and out will come Juliet's father.
They will begin preparations for Juliet's funeral. Thus, you need to go talk to Romeo and make sure he understands the situation, so head on back over to him. After a talk with him, you'll head on down to the crypt with him, and he'll go over to Juliet.
However, Romeo being the genius we've gotten to know him, won't even realise she is still alive. Juliet's cousin comes over to him, and he sees what a fox she is, and decides he'd be happier with her, completely forgetting the love of his life. Whatever.
